For the applicant
- Proof of identity: birth certificate or passport
- PPS number
- Marriage or civil partnership certificate, if applying as a couple
- Proof of every source of income: pension statements, social welfare confirmation, rental agreements
- Most recent tax assessment or P21 statement of liability
- Recent statements for every bank, building society, credit union and An Post account
- Statements for shares, funds, bonds and prize bonds
- Evidence of any money or property transferred in the last five years
For property
- A recent written valuation of the family home from an estate agent
- Valuations of any other property, land or business
- Mortgage statements showing the balance outstanding
- Folio number or Land Registry details, if applying for the nursing home loan
- Rental agreements for any property rented out
For a couple
The same income, savings and property documents for the partner, because the assessment is on the combined figures.
If the applicant cannot sign
- The registered enduring power of attorney, or
- The court order appointing a decision-making representative, or
- Where a family member is applying as a specified person for the main application only, their own identity and relationship details
The nursing home loan cannot be applied for by a specified person; it needs formal legal authority.
Questions families ask
How recent do the bank statements have to be?
The HSE asks for current statements. Six months is a safe rule for every account so that any large movements can be explained.
Who can do the property valuation?
Any auctioneer or estate agent. It does not need to be a chartered surveyor's report.
Where does the application go?
To the Nursing Homes Support Office for the applicant's area. The HSE lists the offices by county.
