Can the applicant understand and sign the application?
Is there an enduring power of attorney?

Their situation

Is the applicant one of a couple?

Married, civil partners, or living together as a couple for three years or more.

Where are they right now?

Income

Tick every source of income the applicant has. For a couple, tick the partner's too.

Savings and investments

Tick everything held, even small amounts. The HSE wants a statement for each.

Has the applicant given away money or property in the last five years?

Gifts to children, paying off someone's mortgage, signing over a house or land.

Property

Does the applicant own their home, or a share of it?
Is there a mortgage or loan secured on it?
Who will live in the house once the applicant is in care?
Would you like to defer the property part of the contribution until after death (the nursing home loan)?
Does the applicant own any of these?

Has a nursing home been chosen?
Is the applicant's spouse or partner also likely to need care soon?

ThenDownload the official application form from hse.ie/nhss, fill it in, and send it with these documents to the Nursing Homes Support Office for the applicant's county. Keep a copy of everything.
